Forensic Engineering: Lessons Learned From Failures

Much of the knowledge and successful engineering used to design, construct, manufacture, and operate engineered facilities and products has been obtained through learning from failures. The interdisciplinary communications about the causes of failures and accidents often results in improved design practices. This presentation will review examples of failures that were the result of variety of causes involving both technical/physical problems and human error/procedural factors.

About Donald W. Vannoy

Donald W. Vannoy received his Bachelor of Science degree in civil engineering from the West Virginia Institute of Technology in 1970. His formal studies were completed with a Master of Science in 1971 and Doctor of Philosophy in civil engineering in 1975, both from the University of Virginia.

Vannoy is a nationally and internationally recognized authority in civil engineering with over 40 years of design experience. He is a professor emeritus at the University of Maryland, as well as the president of the forensic engineering company Trident Engineering Associates, Inc. and the general civil engineering firm Vannoy & Associates, LLC. He is a registered professional engineer in Maryland, West Virginia, Virginia, Louisana, Pennsylvania and the District of Columbia.

Vanoy has conducted over 3,500 forensic studies and failure analysis of projects throughout the country. The projects have included topics involving water intrusion, roofing, construction, structural evaluations, building envelope studies, foundation evaluations, and general civil engineering problems. These studies have included the evaluation and performance of materials including concrete, masonry, wood, steel, aluminum, glass, composites, roofing materials, waterproof coatings, sealers, and other civil engineering materials used in the built environment.
